We are seeking a dedicated and experienced Senior Social Worker to join our Hospital Team in Wakefield, offering a competitive rate of £35 per hour. This role is pivotal in providing high-quality social work services to patients and their families, ensuring their needs are met during hospital stays and facilitating smooth transitions to community care. The successful candidate will be responsible for assessing patients' social care needs, developing care plans, and collaborating with multidisciplinary teams to deliver comprehensive support.

Key responsibilities include:

Conducting thorough assessments of patients' social, emotional, and practical needs. Developing and implementing care plans in collaboration with healthcare professionals. Providing support and guidance to patients and their families throughout the hospital stay. Facilitating discharges and ensuring appropriate community resources are in place. Maintaining accurate records and documentation in compliance with legal and organizational standards.

To be considered for this role, candidates must possess the following essential qualifications and skills:

Qualified Social Worker status with a valid Social Work England registration. A minimum of 3 years of experience in a social work role, preferably within a hospital or healthcare setting. Strong knowledge of relevant legislation, policies, and best practices in social work. Excellent communication and interpersonal skills, with the ability to work effectively in a team. Demonstrated ability to manage complex cases and work under pressure.
