Your key responsibilities will include:

Conducting comprehensive assessments of adults in need of care and support. Developing, implementing, and reviewing care plans in collaboration with individuals and their families. Providing expert advice and support to clients, their families, and other professionals. Ensuring compliance with relevant legislation and best practice guidelines. Maintaining accurate and up-to-date case records and documentation. Participating in multidisciplinary meetings and contributing to team discussions.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in adult social care, particularly with vulnerable populations. Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues. Excellent assessment and analytical skills, with a focus on person-centered approaches.
