Your key responsibilities will include:

Conducting comprehensive assessments and reviews of service users’ needs. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing ongoing support and guidance to individuals and their support networks. Working closely with other professionals, agencies, and stakeholders to ensure integrated service delivery. Maintaining accurate and up-to-date records in accordance with local policies and legislation.

To be successful in this role, you will need to possess the following qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience in adult social care, particularly in community settings. Strong assessment and analytical skills, with the ability to make sound decisions. Excellent communication and interpersonal skills, with a compassionate approach to service users.
