Job Summary

We are seeking a dedicated and experienced Social Worker to join our Community Team. The successful candidate will be responsible for providing high-quality social work services to adults within the community, supporting their well-being and independence. This role requires a strong understanding of UK social work legislation, safeguarding practices, and multi-agency collaboration.

Key ResponsibilitiesConduct assessments of individuals' needs and develop care and support plans in line with relevant legislation and best practices.Work collaboratively with service users, their families, carers, and other professionals to provide person-centered support.Carry out risk assessments and implement safeguarding measures to protect vulnerable adults.Facilitate access to appropriate community resources, services, and support networks.Maintain accurate and up-to-date case records in accordance with legal and organisational requirements.Advocate on behalf of service users to ensure their voices are heard and rights upheld.Participate in multidisciplinary meetings and contribute to the development of policies and service improvements.Comply with the Social Work England standards and adhere to professional ethics and values.Essential RequirementsDegree in Social Work and registration with Social Work England.Proven experience in adult social care or community-based social work.Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.Excellent communication, problem-solving, and assessment skills.Ability to work independently and collaboratively within a multi-agency framework.Commitment to continuous professional development and learning.Ability to manage a complex caseload effectively.Desirable Skills and ExperienceExperience working with older adults, individuals with disabilities, or those with mental health conditions.Knowledge of local authority processes and policies.Familiarity with case management systems and electronic record-keeping.Additional training in safeguarding, risk management, or crisis intervention.

In the first two weeks We would expect the worker to be in everyday to get up to speed.

Thereafter the expectation is for practitioners to be in for one to two days in the office.

An experienced Social worker to work in a busy community team, completing Care Act assessments, carer assessments , reviews, support planning and where needed mental care capacity and best interest decisions. Practitioner are expected to lead with Safeguarding Enquiries and work in partnership with other services such as health, housing and voluntary organisations.

We are seeking a dedicated and compassionate Social Worker to join our Community Learning Disability Team (CLDT). The successful candidate will work with adults with learning disabilities, supporting them to achieve independence and improve their quality of life. This role involves assessing needs, creating care plans, safeguarding vulnerable individuals, and working collaboratively with multidisciplinary teams to provide holistic support.

Key Responsibilities:Undertake assessments under the Care Act 2014 to determine eligibility for support and develop person-centred care plans.Work with individuals, families, and carers to provide tailored interventions that promote independence and well-being.Manage a varied caseload, ensuring timely intervention and appropriate risk management.Advocate for service users, ensuring their voices are heard and their rights upheld.Collaborate with health professionals, housing services, voluntary organisations, and other stakeholders to coordinate care.Conduct safeguarding investigations and implement protection plans in line with Safeguarding Adults policies.Prepare reports and maintain accurate case records in compliance with legislative and organisational requirements.Support service users in accessing community resources and employment opportunities where appropriate.Keep up to date with relevant legislation, policies, and best practices in learning disability social work.Participate in team meetings, training, and continuous professional development activities.Essential Criteria:Degree or Master's in Social Work (or equivalent qualification).Registration with Social Work England.Experience working with adults with learning disabilities and their families.Strong knowledge of Care Act 2014, Mental Capacity Act 2005, Deprivation of Liberty Safeguards (DoLS), and Safeguarding Adults procedures.Ability to complete holistic assessments, develop care plans, and manage risks effectively.Excellent communication, report writing, and interpersonal skills.Ability to work independently and as part of a multidisciplinary team.A commitment to anti-discriminatory practice and person-centred approaches.A valid UK driving licence and access to a vehicle (preferred but not essential).Desirable Criteria:Experience in a Community Learning Disability Team or similar setting.Knowledge of Positive Behaviour Support (PBS) and experience working with individuals with complex needs.Training in safeguarding investigations or Best Interests Assessor (BIA) qualification.

Bolton Council is currently seeking dedicated Adults Social Workers to join a multi-agency Community Learning Disability Team and Intensive Support Team.

We are offering the opportunity to join a focused and dynamic team who are involved in the assessment and development of a wider range of services in Bolton to meet the current and future needs of local people who have a learning disability.

We are a large multidisciplinary team involved in complex areas of work and so there are many opportunities to be involved in formal decision making forums and court processes.

We are looking to recruit enthusiastic and motivated individuals who have a passion for working with adults with Learning Disabilities, understanding their needs, aspirations and supporting them to enhance their quality of life.

The work will involve working with adults with a learning disability within the community to assess and support plan, as well as working with individuals with a learning disability whose needs present as a risk of significant harm to self, or others. This may include individuals who have offending histories, who have, or are at risk of having contact with the Criminal Justice System, or at risk of admission to hospital or in out of borough placements due to the complexities of their needs.

You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and complex decision making.

Key duties -

Care Act assessments and strengths based support planning, taking into account available resources, universal and voluntary services.

Carers assessments
Capacity assessments and Best Interest decision making
Continuing Health Care assessments
Inter disciplinary and multi-agency working.
Safeguarding investigations
Supporting individuals to move into alternative accommodation

Preferred experience of resettlement from out of area placements back into the local area

Experience of report writing such as Ministry of Justice Tribunal reports.

Social circumstances reports for Mental Health Tribunals

Experience of Hospital Discharges

Reports for Community Treatment Reviews.

Court of Protection applications

Qualifications and Experience:

Qualified Social Worker

Experience of complex work with individuals with a learning disability in a statutory setting

Experience of using liquid logic systems would be preferable

Hybrid working - There is an expectation that there will be an initial period of training and induction and to be office based for the first 4 weeks. Following that the expectation would be to be office based for at least 50% of the time
