We are seeking a dedicated and experienced Social Worker to join the Leaving Care Team. The role involves supporting young people transitioning from care to independent adulthood, ensuring they receive the necessary guidance, support, and resources to thrive. You will be responsible for working with care leavers aged 16-25, helping them develop life skills, access education and employment opportunities, and secure stable housing.
Key Responsibilities:Manage a caseload of care leavers, providing tailored support based on their individual needs.Develop and implement Pathway Plans in collaboration with young people, ensuring their voices are central to the decision-making process.Work in partnership with education, health, housing, and employment services to provide holistic support.Offer practical and emotional support to help young people achieve independence.Advocate for care leavers' rights and access to services.Conduct assessments and reviews in line with statutory requirements.Maintain accurate case records and ensure compliance with relevant legislation and policies.Participate in multi-agency meetings and contribute to safeguarding and risk assessments.Requirements & Preferred Experience:Degree in Social Work (or equivalent) and registration with Social Work England.Additional qualifications or training in youth work, mental health, or trauma-informed practice (desirable).Experience working with looked-after children, care leavers, or vulnerable young people, ideally within a local authority or voluntary sector setting.Experience in conducting statutory assessments and developing care plans.Previous involvement in multi-agency working to support young people's needs.Experience in crisis intervention and safeguarding practices.Strong knowledge of relevant legislation, including the Children (Leaving Care) Act 2000.Ability to build positive relationships with young people and empower them in decision-making.Excellent communication, assessment, and problem-solving skills.Ability to work independently and as part of a multi-agency team.A valid UK driving license and access to a vehicle (desirable but not essential)
