Your key responsibilities will include:

Conducting comprehensive assessments of individuals’ needs and risks. Developing and implementing tailored care plans in collaboration with multi-disciplinary teams. Providing ongoing support and guidance to individuals and their families throughout the discharge process. Monitoring and reviewing cases to ensure the effectiveness of interventions and adjusting plans as necessary. Advocating for clients’ rights and access to necessary services within the community.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Proven experience in mental health settings, particularly in hospital discharge processes. Strong interpersonal and communication skills, with the ability to engage effectively with clients and professionals. Knowledge of relevant legislation and best practices in mental health social work.
