We are currently seeking a dedicated and experienced Qualified Social Worker to join our Hospital Discharge to Assess Team in Manchester. As a vital member of our team, you will play a crucial role in facilitating safe and timely discharges for patients, ensuring they receive appropriate support and care post-hospitalization.
Key responsibilities and duties:
Conduct assessments to determine the needs of patients being discharged from the hospital. Develop and implement care plans in collaboration with the multidisciplinary team to support patients' transition back to the community. Coordinate services and resources to meet the identified needs of patients, including arranging for home care, equipment, and other support services. Advocate for patients to ensure they receive the necessary support and services to promote their well-being and independence. Provide emotional support and guidance to patients and their families during the discharge process.Essential qualifications and skills:
Qualified Social Worker with a valid registration with Social Work England. Experience working in a hospital setting and knowledge of the discharge process. Strong assessment and care planning skills with the ability to work collaboratively with other professionals. Excellent communication and interpersonal skills to engage with patients, families, and colleagues effectively. Ability to work under pressure, prioritize tasks, and meet deadlines in a fast-paced environment.Additional requirements:
