We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in the South Neighbourhood of Bolton. As a QSW, you will play a crucial role in providing support and intervention to vulnerable individuals and families within the community.

Key responsibilities and duties:

Conducting assessments to determine the needs and risks of service users Developing and implementing care plans in collaboration with service users and other professionals Providing advocacy and support to empower individuals to make positive changes in their lives Working closely with partner agencies to ensure coordinated and effective service delivery Participating in case conferences and reviews to monitor progress and outcomes

Essential qualifications and skills:

Qualified Social Work (QSW) registration with the Health and Care Professions Council (HCPC) Demonstrated experience in working with children, young people, and families in a social work setting Strong assessment and analytical skills to identify needs and risks effectively Excellent communication and interpersonal skills to build rapport and trust with service users Ability to work autonomously and as part of a multidisciplinary team
