We are currently seeking a dedicated and experienced Adult's Social Worker to join our Transition Team in Sefton. As a key member of the team, you will be responsible for providing high-quality social work services to adults undergoing transitions in their lives, ensuring their well-being and empowerment.

Key responsibilities and duties:

Assessing the needs and risks of adults transitioning between different care settings Developing and implementing care plans in collaboration with service users, families, and other professionals Providing advocacy and support to empower individuals to make informed decisions about their care and support Working closely with multidisciplinary teams to coordinate services and ensure holistic support for service users Participating in safeguarding procedures and promoting the welfare of vulnerable adults

Essential qualifications and skills:

Qualified Social Worker with a relevant degree and HCPC registration Demonstrated experience in adult social work, particularly in transitions and complex cases Strong assessment, communication, and interpersonal skills Ability to work effectively under pressure and manage complex caseloads Knowledge of relevant legislation, policies, and procedures in adult social care
