We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Learning Disabilities Care Management Team in Lambeth. This role involves providing high-quality social work services to adults with learning disabilities, ensuring their needs are met while promoting their independence and well-being. You will work collaboratively with a multidisciplinary team to assess, plan, and implement care strategies that enhance the quality of life for our clients.

Key responsibilities include:

Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and strengths. Developing and implementing person-centered care plans in collaboration with clients, families, and other professionals. Providing ongoing support and advocacy to clients, ensuring their voices are heard in decision-making processes. Monitoring and reviewing care plans to ensure they remain relevant and effective. Maintaining accurate case records and documentation in compliance with regulatory standards.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with adults with learning disabilities or similar client groups. Strong assessment and analytical skills, with the ability to develop effective care plans. Excellent communication and interpersonal skills, demonstrating empathy and understanding.
