The London Borough of Lambeth is seeking a dedicated and compassionate Adult's Social Worker to join our Integrated Team. In this role, you will play a crucial part in supporting vulnerable adults within our community, ensuring they receive the care and services they need to thrive. As an Adult's Social Worker, your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing ongoing support and advocacy to individuals, ensuring their voices are heard. Working closely with multidisciplinary teams and external agencies to coordinate services. Maintaining accurate records and documentation in compliance with regulatory standards.To be considered for this position, candidates must possess the following essential qualifications and skills:
A recognized Social Work qualification (BA/MA in Social Work or equivalent). Current registration with Social Work England. Demonstrable experience in adult social work, particularly in an integrated team setting. Strong assessment and analytical skills, with the ability to manage complex cases.
