We are currently seeking a dedicated and experienced Adult's Social Worker to join the Hospital Team at the City of Wolverhampton Council. As a key member of the team, you will be responsible for providing high-quality social work services to adults in hospital settings, ensuring their safety, well-being, and support needs are met.
Key responsibilities and duties:
Conducting assessments and developing care plans for adults in hospital, in collaboration with multi-disciplinary teams. Providing support and interventions to promote independence, resilience, and social inclusion. Advocating for service users and ensuring their rights are upheld. Participating in safeguarding procedures and risk management to protect vulnerable adults. Maintaining accurate and up-to-date records in accordance with professional standards and regulatory requirements.Essential qualifications and skills:
Qualified Social Worker with a relevant degree and HCPC registration. Demonstrated experience in adult social work, preferably within a hospital or healthcare setting. Strong assessment and care planning skills, with a person-centered approach. Excellent communication and interpersonal skills, with the ability to build rapport with service users, families, and colleagues. Sound knowledge of relevant legislation, policies, and procedures in adult social care.Additional requirements or benefits:
