As part of our dynamic team, your key responsibilities will include:

Conducting assessments to determine the social care needs of patients being discharged from the hospital. Collaborating with healthcare professionals, families, and community resources to facilitate safe and effective discharge plans. Providing information and support to patients and their families regarding available services and resources. Monitoring and reviewing care plans to ensure ongoing support and adjust as necessary. Maintaining accurate records and documentation in compliance with organizational policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly in hospital discharge settings. Strong assessment and analytical skills, with the ability to make sound decisions under pressure. Excellent communication and interpersonal skills, with a compassionate approach to working with vulnerable individuals.
