We are seeking a dedicated and compassionate Qualified Social Worker to join our Hospital Discharge Team in Newcastle. This vital role involves working closely with patients, their families, and multidisciplinary teams to ensure smooth and safe transitions from hospital to home or other care settings. You will play a critical part in assessing needs, planning discharges, and providing ongoing support to promote health and well-being.

Key responsibilities include:

Conducting comprehensive assessments of patients’ social care needs upon discharge. Developing and implementing discharge plans in collaboration with healthcare professionals. Providing information and support to patients and their families regarding available services and resources. Facilitating communication between the hospital team, community services, and families. Monitoring the outcomes of discharged patients and adjusting support as necessary.

To be considered for this role, candidates must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working in a hospital or health setting, particularly in discharge planning. Strong assessment, communication, and interpersonal skills. Ability to work effectively in a multidisciplinary team and manage a varied caseload.
