Join our dedicated Community Team in Bristol as a Qualified Social Worker specializing in Adult Services. We are seeking a passionate and experienced professional to support individuals in achieving their personal goals and enhancing their quality of life. In this role, you will work collaboratively with service users, families, and multi-disciplinary teams to assess needs, plan interventions, and promote independence.
Your key responsibilities will include:
Conducting comprehensive assessments of service users' needs and risks. Developing and implementing tailored care plans in partnership with service users and their families. Providing ongoing support and advocacy to individuals, ensuring their voices are heard. Collaborating with external agencies and community resources to facilitate access to services. Maintaining accurate and up-to-date case records and documentation in compliance with policies.To be considered for this position, you must possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Demonstrable experience in adult social work, particularly in community settings. Strong communication and interpersonal skills, with a commitment to person-centered practice.
