We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Team in Middlesbrough. This role is essential in supporting adults with varying needs, ensuring they receive the best possible care and support. You will work collaboratively with service users, families, and other professionals to deliver high-quality assessments and interventions, promoting independence and well-being.

Your key responsibilities will include:

Conducting thorough assessments of individual needs and risks to develop tailored support plans. Working closely with service users to facilitate access to services and resources. Providing ongoing support and advocacy to empower individuals in their decision-making processes. Collaborating with multidisciplinary teams to ensure comprehensive care and service delivery. Maintaining accurate records and documentation in accordance with regulatory standards.

To be successful in this position, you must possess the following qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly in assessment and care planning. Strong interpersonal and communication skills, with the ability to build rapport with diverse individuals. Knowledge of relevant legislation and best practices in adult safeguarding and mental health.
