We are seeking a dedicated and compassionate Social Worker to join our Adults North and West Community Team 2. In this role, you will play a vital part in improving the quality of life for vulnerable adults within our community. As a Social Worker, you will assess individual needs, develop tailored care plans, and ensure that clients receive the necessary support and resources to thrive. Your expertise will help empower individuals to regain independence and improve their overall well-being.
Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and circumstances. Developing, implementing, and reviewing personalized care plans in collaboration with clients and their families. Providing ongoing support and advocacy to clients, ensuring access to appropriate services and resources. Maintaining accurate and up-to-date records in compliance with local and national regulations. Collaborating with multidisciplinary teams and external agencies to facilitate holistic care.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with the relevant social work regulatory body. Strong knowledge of adult social care legislation and best practices. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
