Description

Our client is looking for an experienced Community Mental Health Care Coordinator, to work with adults with severe and enduring mental health problems in the community - Working within a multi-disciplinary team providing assessment and planning of individual packages of care for service users and carers and promote the independence and well-being of adults.

A key part of the role is to provide overall responsibility for managing individuals through rehabilitation into the community aged 18 to 65 years of age, assessing individual risks, promoting engagement with multi agencies, interventions care planning, case management

I am looking for empathetic, resilient Care Coordinators - Community Psychiatric Nurse, Social Worker, Occupational therapists - who are able to work under pressure and deal with challenging behaviour whilst keep the clients needs as the focus of your work. This will be a fantastic opportunity to make a difference whilst maintaining a work life balance at the same time.

You will have had some recent experience working with an NHS within a Mental Health setting. You will be a qualified Community Psychiatric Nurse (CPN) Social Worker (QSW) or Occupational Therapist (OT) be fully HCPC or NMC registered and ideally have a current DBS (within 12 months) however Caritas will carry one out on your behalf if required.
