Older Persons

The main focus of Social Workers within these teams is to devise care plans for elderly service users which may include services addressing issues surrounding personal care, mobility, social isolation and mental health concerns. Assistant Social Workers/ Care Managers work alongside the Social Workers to support independent living. If this is no longer possible, they will assist them in moving to residential or nursing home care.
