Leaving Care

Leaving Care Social Workers work with young people leaving residential or foster care placements and offer support through their transition towards independent living. Pathway plans are drawn up based on the outcomes of an assessment of factors including the young person’s health, educational training / employment needs, financial needs and support available from existing family members. These plans set out the advice, support and assistance which will  be provided by the local authority through this period of transition. Outreach/ key workers work alongside the young person, meeting regularly to ensure that all emotional, physical, educational and financial needs are met. The pathway plans are monitored and reviewed where necessary.
